Yeesh, story of my life ...
Well, if it's my grandmother or great-aunt saying something racist or homophobic, I ignore them and then angst about it later. It's probably not something I'm going to be able to change by arguing with them about it. Plus, it's my grandma ... you don't call your grandma out on something.
If it's a friend, or an acquaintance, you bet your ass I call them out on a racist or homophobic comment. It doesn't matter who's around or whether it's directed towards me: it's hurtful, it's wrong, and they need to stop.
The phrase 'that's gay' is frequently the subject of my pointed look and annoyed cough. Most people who know me know to retract their sentence and say something else by now.
